Student Health Services

Golf School District 67 provides health services at both Hynes Elementary School and Golf Middle School. Qualified health staff help protect students from disease and administer first aid. Health staff also provide direct services to students with significant medical needs.

In order to protect their own well-being and the well-being of other students, all students are encouraged to have annual physicals and ensure that students are up-to-date on their vaccinations. Please refer to the district's Health Requirements section for more detailed information.

Parents must advise the health services personnel if their student requires special medication or has health issues and/or critical health needs.

Students who want to participate in extra-curricular activities must submit the appropriate health forms, completed by a physician.

Student Health Requirements

When should students stay home sick?

Students should not attend school if any of the following conditions are present:

  • Temperature of 100.0 F degrees or higher
  • Vomiting
  • Diarrhea
  • Incessant or deep cough, and/or purulent discharge from the nose
  • Red eye(s) with discharge and/or crusting on lashes and pain or irritation
  • Any skin lesions that are open and weeping or an unexplained rash
  • Contagious illnesses
  • Obvious signs that your student cannot comfortably participate in class activities (even without a fever)

Symptoms should be absent before the student returns to school. Children with communicable diseases will be excluded from school. Please contact your physician in these cases, and notify the nurse or administrator when a diagnosis is established.

Submitting Health Forms

Once completed, forms can be submitted by:

  • Faxing to the student's school
  • Dropping off at the student's school
  • Emailed to the student's school nurse
  • Mailed to the student's school

Care Plans

Students with chronic medical conditions must submit a care plan to the school’s nurses’ office.

All medications listed on a care plan must have a Medication Authorization on file. 

For your convenience, care plans for common conditions are provided.

A student’s health care provider may use their own plan form if preferred. 

The Care Plan is required to be completed annually for students with a chronic medical condition.

Waivers and Exemptions

A 6th-grade or transfer student’s certificate of child health examination (physical) meets the requirement of a pre-participation examination (athletic physical) if the student participates in a school-sanctioned athletic team. Once submitted and approved, the athletic teams will access the physical for athletic eligibility purposes. 

An examination is valid for 364 days from the date of completion. For a student to be eligible to participate in an athletic school program, they must have a valid physical on file. During a student's 7th or 8th-grade year may elect to have a pre-participation examination (athletic physical) completed instead of a child health examination (physical).
